Mother's Day Out

The Faith Seeds Mother’s Day Out program at Faith West accepts children ages 18 months through 4 years. Please complete this form for a tour of our facility.

To register, pleasedownload the following application packet (PDF). Payment of $75.00 non-refundable registration fee is due with the registration forms.

We accept on a first come, first serve basis and have a limited number of spaces available in each class. Your child’s spot will be secured when we have received the completed paperwork and registration fee. The completed packet may be dropped off at the Welcome Desk of the main school building, or can be mailed directly to the school. Our mailing address is:
